Support at Home After Hospital

Our support at home services provide a lifeline to vulnerable and older individuals in the local community. We help people regain confidence and independence after a hospital visit, connect them to other support services, and prevent the need to move into permanent residential care or additional hospital stays. please read below to find out more about our support at home services. 


Take home and settle 

Take home and settle is a free service that provides transport to take vulnerable people aged 65 and over home from Worthing hospital. We assist with unpacking their bag, checking the food in their fridge is still fresh and doing a small shop if required. Additionally, we offer referrals to other support services when required.


Support at Home after Hospital

Our team support people for up to six weeks following a hospital discharge to regain confidence and independence. This service offers companionship visits, domestic support, shopping, picking up prescriptions and signposting.  

A support at home after hospital volunteer can support with a range of tasks including:

•    Collecting prescriptions and basic shopping
•     Providing information on other services or equipment you might need
•    Companionship
•    Escorted walks
•    Engaging in local activities

(Please note that this service does not offer personal care, nursing/clinical care, dispensing medication, counselling, or domestic cleaning.)

What is provided?

Up to six weeks practical support after discharge from hospital for local residents in West Sussex.

Guild Care is working in partnership with Age UK West Sussex Brighton & Hove and Carers Support West Sussex to provide a countywide service, with Guild Care delivering the service in Adur and Worthing.

The service supports

Even if you have already made arrangements such as home help or personal care, our Support at Home After Hospital volunteer could:

  • Older people
  • Vulnerable adults
  • People living alone
  • Anyone about to leave hospital or who has recently been discharged (aged 65 and over)

How we can help

  • We will visit and make sure you are safe, have what you need and that the relevant services are in place should you need them.
  • We will ensure you are not isolated following your return from hospital
  • We will visit to help you regain your confidence and independence.

Costs

There are no charges for these services as they are supported by West Sussex County Council.
